To Heart 2 is 4:3, translated, and a classic from 2005, but I'm not sure it fits many of your other criteria.

I came down with a flu today and as I don't have many resposibilities ATM, decided to spend the next few days catching up with anime. After getting to the current episodes in simulcasts, I've resumed watching Yanada-kun and the Seven Witches which I stalled a while ago and damn, so far it's hands down the best ecchi comedy I've ever seen. Extremely fun and quite cleverly-written, with the kissing scenes (the premise is that the protagonist randomly discovers he has magic powers that work throught a kiss) and mild fanservice being kind of constantly present, but never vulgar or coming out of nowhere... Why can't more shows be like that?

Rewatching Little Witch Academia with the family. What great fun this show was. Has anyone noticed how amazing the soundtrack is? It sounds like something from a feature length film.

You've already read To Heart 2 and given it a 9. You have good taste but I don't have anything more to recommend.

I'd go beyond that and say sometimes the translator even asks the editors for ideas while they're translating on how to tackle some issues where there is no good translation for a word, name, pun, meme, etc.

    Absolutely.
I always work closely with the editors on my projects, and have never been in a situation where that wasn't possible. Editors should always be able to ask the translator for their opinion on something / ask for further explanations of translations should they need it, and translators should always read the editor's edits in case there is something they feel should be handled differently. (And in these cases, the editor and translator will often discuss how to handle it together, like Fred pointed out.)

So, this is very much not the case, at least in my experience:

Can only speak from my personal experience, which is about a dozen things with about eight different translators, but at least from what I've seen in the industry, the translator usually checks over the editor's output and does so-called "counter-edits" (75% of the games I've worked on). Also, who gets the "final cut" between the TL and the editor is kind of up in the air, though usually the TL and editor simply discuss anything where they disagree, so it's not really an issue.
And just briefly, regarding Fureraba: the translator has spoken very highly of the work the editor did on Fureraba on several occasions, to me and to others, so you can at least assume they were agreed that it was good work overall. I haven't read it myself, but the screenshots I've seen posted here and there have always looked fine to me. ¯\_(ツ)_/¯

The text is long and presents a lot of unique challenges, the artform is lowbrow, the pay is shitful, the deadlines are punishingly short, and some (not all) of the audience is hypercritical and ungrateful. If you were a half decent translator, of all the translation gigs in the world why would you do it unless you loved VNs? Additionally since the pay is so bad one can only earn a living off it by doing vast chunks of stuff they don't like at all. I know many translators that started out loving VNs and translating them has killed their love of them. Luckily I can afford to just do it for fun and will only work on VNs I find appealing in some way. Not everyone is so lucky.

Ungrateful was with respect to fan translations. If you pay for something you have the right to get what you paid for, but the problem is there are so few translators they get worked too hard with too short a deadline and in that scenario it's not their fault if quality drops off. Solpress does indeed have the honorific toggle option - which means we have to do even more translation for the same (very low) fee by the way. I'd love to translate in English instead of American, but that's not up to me. As for changing units, I flatly refuse to do that.

The problem is that you can't actually get a lot of good criticism. Usually it's 99% vague autistic screeching. With that said I'd love for that autist anon on 4chan to go through one of my translations in case I let some mistranslations slip through, I can just ignore whatever things I don't care about in his evaluation. That's only the reading-the-japanese side of translation though which is not that big of a part of it. It is incredibly rare to get critique on an actually useful level, and it's certainly not going to come from the fans who actually get upset about most translations.
